Participation Acknowledgement
I understand that Made to Be workshops include hands-on arts and crafts activities using age-appropriate materials and tools. I understand that, as with any hands-on activity, minor accidents or injuries may occur.
I have provided any allergies, medical conditions, accommodations, or other information that Made to Be should reasonably know in order to support my child's safe participation.
I give permission for my child to participate in the selected Made to Be workshop and acknowledge the ordinary risks associated with these activities.
